أَخْبَرَنَا مُحَمَّدُ بْنُ عَبْدِ اللهِ بْنِ بَزِيعٍ قَالَ حَدَّثَنَا يَزِيدُ وَهُوَ ابْنُ زُرَيْعٍ قَالَ حَدَّثَنَا شُعْبَةُ عَنْ أَبِي إِسْحَاقَ عَنِ الْبَرَاءِ
أَنَّهُ سَمِعَ النَّبِيَّ ﷺ يُوصِي رَجُلًا إِذَا أَخَذَ مَضْجَعَهُ أَنْ يَقُولَ «اللهُمَّ أَسْلَمْتُ نَفْسِي إِلَيْكَ وَوَجَّهْتُ وَجْهِي إِلَيْكَ وَفَوَّضْتُ أَمْرِي إِلَيْكَ وَأَلْجَأْتُ ظَهْرِي إِلَيْكَ رَغْبَةً وَرَهْبَةً إِلَيْكَ لَا مَنْجَى وَلَا مَلْجَأَ مِنْكَ إِلَّا إِلَيْكَ آمَنْتُ بِنَبِيِّكَ الَّذِي أَرْسَلْتَ وَبِكِتَابِكَ الَّذِي أَنْزَلْتَ فَإِنْ مَاتَ مَاتَ عَلَى الْفِطْرَةِ»
Translation

✧ He heard the Prophet ﷺ advising a man, when he took to his bed, to say: "O Allah, I have submitted my soul to You, turned my face toward You, entrusted my affair to You, and leaned my back upon You, out of desire and fear of You. There is no refuge or shelter from You except to You. I believe in Your Prophet whom You sent and in Your Book which You revealed. If he dies, he dies upon the fitrah."
